• Fill out the form.
• Attached a voided check for the account you want funds transferred from.
• Mail both to: Durango Dharma Center, P.O. Box 1631, Durango, CO 81302

Choose the amount you want to contribute monthly and it will be automatically debited from your checking account. Remember, you can sign up, change or cancel your Direct Deposit at any time.
